Eligibility

  • HEART grants are available only when all other sources of funding have been exhausted. Some examples of alternate resources include insurance, county and/or state funding, family assistance, personal assets, etc. Your staff is expected to investigate and make a determination of need. Need is defined as: without our help, the client is unable to enter treatment, fully participate in, or complete their treatment program. Please do not submit applications you believe will be denied or if the client is not in dire financial need.
  • Applicants must be in an AFFILIATED facility for primary treatment (can be in-or out-patient, 1/2 way or 3/4 housing) AT THE TIME THE REQUEST IS REVIEWED. Extended care and aftercare are not considered primary care. The client does not qualify for HEART funding for treatment costs if insurance or public assistant programs will cover 65% or more of treatment costs (pre-discounted rate). The client can still request funding for non-treatment expenses, such as sober housing.
  • Clients may only apply for funding once per treatment cycle, regardless if a previous application was approved or denied.
  • A grant request will be denied if the applicant previously received a grant from us and made no effort to give back to HEART through repayment (partial or full) or volunteering.

The Review Process

Applications are reviewed on an ongoing weekly schedule as follows:
Monday 5:00pmDeadline to be reviewed that week. Applications submitted after that time will be submitted the following Wednesday for review.
Wednesday 8:00amApplications are emailed to the Board for Review. Deadline to supply any missing information.
Thursday-FridayDecisions have been made. The Program Coordinator will call the counselor/contact listed on the application with the results.

  • A counselor must review and submit the application to HEART, the application should not be submitted by the applicant.
  • A counselor must fill out page 3 of the application.It is important that the applicant’s counselor go over the application before it is submitted. The counselor's care in checking the application for completeness will make it possible for HEART to provide a prompt answer to your client’s request for financial aid. Note that the application's review may be delayed up to a week after submission due to missing information.
  • Answer all questions to the best of your ability. If the answer is "$0" you can leave it blank. Round off dollar amounts to the nearest dollar.
  • Be sure to keep a copy of the completed application for your records and for future reference. (A copy of the completed application will be forwarded to you after it is received by the Program Coordinator)
  • If you expect to be unreachable by the phone number you have provided, please leave the contact information of a second person at the facility to answer any follow-up questions.
  • Please refrain from referring your clients to HEART to check the status of their application, ask for an application and / or question HEART funding decisions.

Disbursement of Approved Funds

  • The applicant’s counselor, or requested contact person, will be notified by phone and with a follow-up confirmation letter whether a request has been approved or denied by the HEART Board of Directors.
  • Payments are NEVER made directly to the applicant.
  • Funds for treatment costs (inpatient or outpatient) are paid upon receipt of a bill to HEART The client must have completed at least 2/3 of their program for bill to be paid.
  • Payments for all other non-treatment costs are made directly to the creditor immediately upon approval of the grant. HEART will require all necessary account information – company name, contact, amount, account number, complete address and telephone number. Bills for medical, dental or eye care work are paid from either a written estimate or upon receipt of a bill from the doctor’s office.

Approved Uses of HEART Funds

INPATIENT TREATMENT - (must consist of 4 or more hours a day of program.)
Maximum grant amounts:
$2,000.00 for 20 or more days of inpatient treatment
$1,000.00 for 10 – 19 days of inpatient treatment

  • The client must complete 2/3 of treatment to receive funding.
  • Affiliated facilities are required to match or exceed the requested grant amount in free care.
  • If insurance or public assistance programs cover more than 65% of treatment costs, HEART will not cover any remaining treatment costs.

 

OUTPATIENT TREATMENT - (3 or more hours of program a day, 3 times weekly, 50-hr minimum.)
Maximum grant amount: $750.00

  • The client must complete 2/3 of treatment to receive funding.
  • Affiliated facilities are required to match or exceed the requested grant amount in free care.
  • If insurance or public assistance programs cover more than 65% of treatment costs, HEART will not cover any remaining treatment costs.

 

1/2 WAY OR 3/4 WAY HOUSING - (a supervised, structured, sober living environment with programming.)
Maximum grant amount: $750.00 for one month of halfway or three-quarter housing costs

  • Affiliated facilities are required to match or exceed the requested grant amount in free care.

 

OTHER  REQUESTS - HEART will consider any combination of the following requests, not to exceed $600 total.

  • Sober Housing - Does not include security deposits. First month only.
  • Transportation Costs - for client or family
  • Rent / Mortgage /Utilities - Bills that are past due before client enters treatment will NOT be considered.
  • Medical / Dental / Eye care - Bills that are past due before a client enters treatment will NOT be considered.
  • Daycare / Babysitting
  • Insurance/Cobra Payments - Bills that are past due before entering treatment will NOT be considered.
  • Room and Board Family
  • Other (e.g., storage) - Bills that are past due before entering treatment will NOT be considered
